It is our goal to make accessing any forms required for completion to be faster and easier by giving you online access to complete & submit.

MEDICAL FORMS

You will find all of the required medical forms under each of the following headings. Please print all of the forms for your child’s medical condition, complete, sign and submit them to the school. Please note that some of the documents require a doctor’s signature.



 
Why are these forms required? 
Below is a description of each form and why it is required.

Administration of Oral Medication
This form is required if your child's doctor has prescribed medication that your child is required to take during school hours, this includes oral allergy medication and asthma inhalers. This form requires a doctor to complete a section and sign off on the medication. If using this form for prescription antibiotics, we recommend that you ask the pharmacy to prepare two bottles of liquid medication, enough for any doses for school and the remainder to be kept at home. This form is also required if your child keeps Benadryl on other oral allergy medication in the school office for allergies. This form is required annually or per oral medication.

Serious Medical Condition
This form is required if you child has a serious medical condition. Please complete and sign this form annually and return it to the school office.

Severe, Life Threatnening Allergy
This form is required if your child has a severe, life threatening allergy. Please complete and sign this form annually and return it to the school office.

Administration of EpiPen
This form is required if your child keeps an EpiPen in the school office and/or on them/in their backpack. This form requires a doctor to complete a section and sign off on the medication.  This form is required annually.

OSTA Medical Form
This form is required for all students with medical conditions, as students may also take transportation for school field trips.
